Adrard's Townhouse is a home that belongs to Sabione Adrard, accessed through the back of her shop. It is built against the ancient city wall, and is linked to tunnels leading into the Palace District.
During the Worm Cult attack, this fact makes it important, and you can only access the building when given Adrard's Townhouse Key. Entering through the shop, you will find yourself on the top floor, and you will have to go downstairs to reach the guarded hole in the wall.
