Reference cleanup

Split up the references. Should be easier to locate now. As mentioned in the edit the information is mostly from Hidden and Calamity. Stuff about how the layers work is Leramil and Meln in the former, and Blightcrown's texts. Stuff about the effect on Apocrypha and Mora and the Daedra is Mora and Scruut from the latter, and Blightcrown's texts. Meln during Conclave for all the Telvanni stuff. Shelreni during Calamity for her own role, and Blightcrown's texts and her for the mix of disease and dreamstuff (though Leramil also repeats this).

Spread of the disease is also explained by Leramil during Unhealthy Fate Calamity, and before that she reiterates the mix of dreamstuff and disease and their concealing effect during Calamity.

Ghost Sight stuff is Meln early on (Spirit of Fate) and the rest is from how you use the Ghost Sight in game (using it to find the Censers in the panopticon or to counter Vaermina's shifted Daedra).
